I see my doctor already. Do I need this?

Yes. This visit complements the care you receive from your Primary Care Physician (PCP). The Nurse Practitioner conducts a comprehensive assessment and provides a summary to your PCP, ensuring they have current and complete information about your health status. This supports care coordination, helps identify any unmet needs, and allows your PCP to plan appropriate follow-up care.

What happens during the visit? What does it involve?

The visit is conducted by a Nurse Practitioner and typically lasts about 45 minutes. During the assessment, the provider will review your medical history, current medications, and overall health status to develop a comprehensive clinical summary. This information will be shared with you and your Primary Care Physician (PCP) to support ongoing care and coordination. The provider may also discuss available care programs and services that can help you better manage your health.

What should I expect after the visit is completed?

Members will receive a phone call within 10 days of completing their visit with our Nurse Practitioner to complete a brief satisfaction survey to support quality improvement efforts. A written summary of the visit will also be mailed for the member’s records and to share with their Primary Care Physician (PCP) to ensure continuity of care. Approximately 30 days following the visit, a follow-up call may be conducted to confirm PCP follow-up and facilitate appointment scheduling, if needed.

Who will be conducting the visit?

The visit will be conducted by a licensed Nurse Practitioner, a qualified clinical provider trained to perform comprehensive health assessments and support your overall care.

How does a telehealth visit work?

This visit is conducted virtually through a secure video call, accessed using a simple link with no app download required. The video format allows the Nurse Practitioner to see and interact with you in real time, which is an important component of completing a comprehensive health assessment. During the visit, the provider will perform a visual evaluation and review your health status, and you will have the opportunity to ask any health-related questions.

What does this cost, and will it affect my coverage?

There is no cost to you for this assessment. It is covered by your insurance as a preventive service and does not affect your benefits or coverage. This assessment is designed to support your health and help ensure you receive appropriate preventive care.

Yes. This is an annual assessment that helps ensure your health information remains current and allows your care team to identify any changes, address potential risks, and support appropriate care planning each year.
